Please complete the appropriate PDF application below.  Applications can be completed by typing your information online.  There are several methods to submit your completed application:

1. Via email (the EMAIL button is located at the bottom of the form).
2. Fax it to 303.779.4834.
3. Mail it to ASBP.

• Physician Membership - For legally practicing MDs or DOs.

Bariatric Practice Guidelines Oath

• Associate Membership - For non-physician health care providers (i.e. PAs, RDs, NPs, RNs).

   

• Affiliate Membership - For those interested in bariatrics, but without a health provider education/degree (i.e. administrative staff in bariatric practices).

   

• Physician-In-Training Membership - For MDs or DOs currently enrolled in an approved internship, residency or fellowship program (proof of status required).

   

• Student Membership - For any medical student attending an accredited school (proof of status required).
